Transition

From the year 9 Annual Review, transition will become a focal point in relation to provision post 16. New Options (Higford) School will formulate a transition plan, taking into account the views where appropriate of the pupil and their family.  Social services, The Local Education Authority, The Health Authority and Connexions will also be invited to contribute to the transition plan where appropriate. This is then reviewed annually at the review up to the end of Key Stage 4. The aim is to aid a smooth transition to an appropriate service to support future developments of the pupil.

Post 16 at the New Options (Higford) School will be a three year phase of transition and during this time we see children develop into adults. Transition planning is key to the success of this phase, and throughout the three years support will be provided to secure appropriate future placements for our students and ensure smooth transition to adulthood.  Collaboration with parents and outside agencies is paramount and this will include social services and Connexions as appropriate.

For some students at the end of compulsory education, transition is likely to be to an AALPS College with an individually supported programme of life, leisure and vocational skills, whilst for others a specialist college programme and/ or supported employment with residential development towards supported living for adults with autism and/or complex sensory and communication needs might be more appropriate.
