Following receipt of an initial referral, interviews are arranged with the young person and their circle of support (if appropriate) to assess needs.
A comprehensive assessment is compiled which forms the basis of any subsequent transition plan.
There may be further processes to undertake depending upon the source of funding.
Should an application be successful and all necessary requirements met in relation to the eligibility criteria, a placement would be offered and arrangements made to facilitate transition.
