Options Outreach is a highly successful service located in the West Midlands providing people aged 16+ with individually tailored care pathways to independent living. From transition services for those leaving residential care to supported living services for those ready to make that step, Options Outreach focuses on promoting positive community integration.
As a key part of the BESD journey of care, the service enables young people who are leaving care to make the transition to independent living with the security of planned, consistent and comprehensive support.
By undertaking transition planning as early as possible and involving each person and their circle of support, we aim to make the transition from children’s services to adult services an enjoyable experience.
As part of the leaving care and supported living assessment processes Options Outreach addresses many factors including housing provision, length of stay, the level of support required, training and employment, and individual budgets and benefits. Packages of support are reviewed on a regular basis to ensure that they remain proactive and purposeful.
During assessment Options Outreach works in partnership with the person requiring the service and their circle of support to ensure that they are able to choose the right package for them.
